Admission Criteria

For admission to St. Ann’s Residence, prospective residents must

-Meet the age requirement of 62 years and above, or 55 years old and disabled.

-Must have a History & Physical within 3 months prior to moving to St. Ann's.  Prospective resident must also supply a copy of the report to one of the RN's at St. Ann's.

-Agree to provide the information requested in the Admission Packet.   

-Agree to abide by all terms set forth in the Lease Agreement. 

-Agree to abide by all the terms set forth in the Resident Handbook.

-Be free of communicable disease which cannot be effectively treated or safely contained at St. Ann’s, posing a risk of infection to others.  Prospective resident must provide documentation of having Tuberculosis testing done within 3 months of moving to St. Ann's. 

-Be independent or secure home health services. 

-Must be assessed by one of our Registered Nurses.
